At Dementia Concern, we provide a monitoring and support service for people with dementia who live alone. Those eligible for this service will be assigned a community support worker (CSW), who will continue to support the person.
Dementia advisers are named workers for people living with dementia. They provide ongoing assessments and provide advice, information, support, monitoring and client representation as appropriate.
Our Dementia Link Workers (DLWs) work alongside the GP Networks in Ealing and the West London Health Trust. They support people (with and without dementia medication) who have been transferred from the cognitive and dementia services with a diagnosis of dementia.
