Dementia Concern is deeply saddened by the death of Her Majesty Queen Elizabeth II.

Our heartfelt condolences are with the Royal Family at this time.

Dementia Concern is contracted by Ealing Community Partners/West London NHS Trust, to deliver dementia services in Ealing. There are two data base systems in use. SystemOne is for contract reporting and Charity Log is the internal database for clients. The charity holds client data on both SystemOne and Charity Log. The purpose of the holding this data on both systems is to provide an adequate service to the clients. The legal basis for the charity holding this data is that of Data Controllers, as the charity provides direct care to patients, and therefore are able to determine the use and purpose of the data that is held. If a service user has any queries regarding the processing of their data they should contact [email protected] The retention period for this information is seven (7) years from when the client last uses the charity’s services. When obtaining consent from clients, the staff aim to highlight to the client that their data is held on two database systems, SystemOne and Charity Log.

Our deep-rooted passion means we are committed to constantly improving our services and expertise in order to provide the best person-centred care possible.
Social Clubs

Our social clubs consist of Weekend Day Care and a Thursday Group for more active people with dementia. They offer people with dementia a chance to socialise in a safe environment and offer respite for their carers.
Dementia Link Workers

Dementia Link Workers work alongside GP Networks in Ealing and the West London Health Trust. They provide ongoing support and advice to people with dementia who are on medication and experiencing cognitive difficulties, as well as empowering carers and families. Support includes regular assessments and referrals to other services.
Dementia Adviser Service

Dementia advisers are named workers for people with dementia. They're the first point of contact for people with dementia, their carers and other professionals who may need assistance to access Dementia Concern. They complete home visits to assess the needs of person with dementia and their carers, while providing advice and information.
Donate

Dementia Concern supports over 1,300 clients living with dementia and in excess of 945 carers. The charity has recently had funding cut from the local council which helped provide our social clubs and respite care at reduced rates.
Our Story

Since 1992, we have been providing information and support services for people with dementia and their carers in the London Borough of Ealing.
Community Support Workers

A monitoring and support service for people with dementia living alone. Community support workers (CSWs) stay with each case from beginning to end, providing emotional and practical support via phone calls and home visits. They regularly monitor the situation to ensure relevant services are in place and to help the person with dementia to attend key appointments.
